The Utah Million Hearts Coalition, in conjunction with the national Centers for Disease Control Million Hearts® initiative, aims to prevent heart attacks and strokes. The coalition encourages and recognizes excellence in hypertension management.
The coalition has a statewide reach through the involvement of Utah Department of Health and Human Services, small and large health systems, local health departments, quality improvement organizations, libraries, community based organizations and federally qualified health centers.
The coalition focuses on improving the accuracy of blood pressure measurement and control by encouraging the use of best practices. The coalition helps to develop and disseminate resources and information to coalition members and clinics that focus on hypertension management and control which are available on this page.

Launching January 2026!
The Clinic Learning Community brings together outpatient care teams working to improve hypertension and chronic disease management.
Through a series of sessions, clinics come together to share what’s working, talk through challenges, and learn from each other in real time. Recent sessions have focused on practical topics like using EHR data, identifying care gaps, and building workflows that fit into busy clinic settings.
The current series runs from January from June 2026 and is supported by Comagine Health coaches. The goal is to create space for honest conversations and help clinics take small, manageable steps toward improving care.
Social Services & Support Needs
The Social Services and Support Needs (SSSN) workgroup focuses on bridging the gap between clinical care and the daily resources required for heart health.
The workgroup is currently refining a centralized resource system to better connect individuals with housing stability, reliable transportation, and food security. Through direct community feedback and partnerships with local health departments, the group identifies common barriers to care. The goal of this initiative is to create seamless, dependable pathways that link clinical settings to real-world community support.
Self Measured Blood Pressure
The Self-Measured Blood Pressure (SMBP) workgroup supports the use of home blood pressure monitoring as part of everyday care.
Current efforts include building a centralized toolkit to help local partners start and maintain SMBP lending libraries, along with expanding access to devices through community locations across the state.
The group is also focused on making sure that home monitoring connects back to clinical care, so patients aren’t just tracking their numbers but getting the support they need to act on them.

Use this interactive map to find a participating location near you where you can borrow a free blood pressure monitor for home use.
Finding a Location Near You:
- Search: Click the Magnifying Glass icon (bottom right) and enter your zip code, city, or address to jump directly to your area.
- Zoom: Click the + (plus) or - (minus) buttons in the bottom right corner to zoom in and out for a closer look at specific neighborhoods.
- Pan: Click and drag your mouse anywhere on the map to move around and view different counties.
Viewing Program Details:
- Select a Location: Click on any yellow heart or teal circle on the map.
- Read the Details: A pop-up window will appear with information about that specific program, including the facility's name, address, contact information, and how many blood pressure cuffs are currently available.
Helpful Map Tools:
- Reset Map: Click the House icon (bottom right) at any time to zoom back out to the main map frame showing the entire state.
- Fullscreen: Click the Expand icon (the four arrows above the magnifying glass) to make the map fill your entire screen for easier viewing.
- General Info: Click the "i" icon (top left) for more general information about the lending program itself.

We will work with local health departments, libraries, and other public centers to get a program established in your area. In the meantime, many local pharmacies offer free blood pressure checks at their in-store kiosks!
Utah Million Hearts Award
Each year, the Utah Million Hearts Coalition honors primary care clinics for their efforts to effectively measure and control blood pressure for their patients. In 2026, 65 primary care clinics were recognized with the Excellence in Blood Pressure Measurement and Control Award. These clinics have met the highest standards of blood pressure measurement and control through proven evidence-based strategies, such as patient education on how lifestyle choices impact blood pressure, precise measurement of blood pressure, engaging patients with a treatment plan to improve medication adherence, fully utilizing the electronic health record for hypertension care and working as a team to ensure positive outcomes for patients with hypertension.
