Generic individualized healthcare plan (IHP):
This generic form helps school nurses create an individualized healthcare plan to guide nursing care and the delegation of medical tasks for students. It does not include diagnosis-specific guidance, since plans for asthma, allergies, diabetes, and seizures are available in their own sections of the website. This IHP is a clinical planning tool for nurses and is not shared with school staff. State law requires an IHP for all students who receive delegated medical care.

Generic emergency action plan (EAP):
Use this form to share instructions with school staff on how to care for and respond to emergencies for a medical diagnosis that is not covered by another condition-specific form. Use to send information to school staff about the student.
Medical diagnosis form:
Use this form only if the healthcare provider has not already provided documentation that includes a medical diagnosis. It is required just once and does not need to be updated each year.
